Mandatory Contractor Profile Requirements

• Proven track record of high quality work

• Billing submitted within 15 days

• All necessary licenses and insurance coverage, including General Liability ($1 million dollars per occurrence, $2 million dollars aggregate) and Workers Compensation (regardless of state requirements).

The contractor application is intended for the sole use of new contractors to ONE. If you are currently providing services for ONE please DO NOT submit an application. Only submit one Application. If you provide services that cross service offerings please fill out the application that reflects your primary business line and fill in other service offerings in the “additional services” section of the application.
